The first MDX was introduced in October 2000 as a 2001 model. It was a car-like crossover built on Honda's global mid-size platform, shared with the Accord, TL and Odyssey, and powered by a 3.5 litre V6 with VTEC. Its VTM-4 automatic four-wheel-drive system had a lock mode for low speeds. It held seven passengers, though the two third-row seats were small. The engine gained power in 2003 and again in 2004, when side curtain airbags and new lights arrived. It was built in Alliston, Ontario.
