The 1991 Aerostar was in its sixth year and carried the 3.0 liter V6 and the 160 horsepower 4.0 liter V6 introduced for 1990, with the E 4WD all wheel drive system available on XLT and Eddie Bauer trims, and a standard and extended length body. The grille was slightly changed in colour from 1989 to 1991, before the second and most substantial exterior update arrived for 1992. Its bodies came in a standard length and an extended length from 1989, as a passenger wagon or a cargo van.
